We will gladly check your OUT of network benefits and will bill your insurance directly if applicable. Please email with your Member ID and date of birth.

CANCELATION POLICY: Your appointments are very important to us, and it is reserved specifically for you. Therefore we adhere to a strict 24 hour cancelation policy. Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and patients on our waiting list miss the opportunity to receive services. Please understand that if you miss your appointment, need to cancel within 24 hours, or arrive later than 15 minutes to your scheduled time, you are responsible for the entire fee of the service. This also applies to insurance patients as we cannot bill your insurance for missed appointments. At the time of booking, a credit card will be required to hold the appointment, however, will not be charged until the time of service.
